Strong communities begin with strong families. UCN’s Community Resource Navigators and certified Community Health Workers work directly alongside parents and caregivers to identify barriers, connect them with services, and create pathways toward economic independence.

 

Our navigators provide personalized, relationship-based support — not just referrals. They sit with families, complete applications together, gather documentation, and follow up to ensure services actually connect.

 

Through partnerships with service agencies across Houston, UCN ensures families receive coordinated, wraparound support tailored to their specific needs. Our trained Community Health Workers bring a trusted, culturally competent presence into neighborhoods that have historically lacked access to health resources.
